Adult Cardiac Arrest Management (BLS)

Basic Life Support for cardiac arrest in out-of-hospital settings

⚠️ Do NOT start CPR if: Valid DNR order OR patient is "obviously dead"

Obviously Dead Criteria

Priority Actions

  1. Call dispatch or 911 for assistance
  2. Assess ABC and confirm pulselessness
  3. Start CPR and attach AED
  4. Confirm type of arrest (medical, trauma, hypothermia, foreign body)
  5. Take brief history from bystanders if available

Timeline Information to Gather

Treatment by Scope

SFA, FR, EMR, and up

  1. Start CPR and attach AED pads; follow commands
  2. Insert oral airway
  3. 30 compressions to 2 breaths

PCP, RPN, LPN, RN, and up

  1. Start CPR, attach AED or manual pads & monitor
  2. Analyze rhythm; if VT or VF, charge and shock
  3. Continue compressions during charging, then shock
  4. Perform high-efficiency CPR, minimize interruptions
  5. Analyze rhythm every 2 minutes
  6. Defibrillate in escalating doses (see below)
  7. Insert oral or nasal airway ASAP - 30:2 ratio
  8. Insert King Tube if prolonged arrest - switch to 1 breath every 6 seconds

ACP, RN (EM/CC)

Follow current ACLS protocols/HLC directives

Manual Defibrillation Doses

Zoll 120J → 150J → 200J
Lifepack 200J → 300J → 360J
